Steps for Requesting an Appointment:

1. Pay a $20 monthly fee to participate in our telemedicine addiction treatment program.
2. You'll be redirected to submit your intake documents after  your payment has been processed.

We will review your submission and contact you to schedule your appointment as soon as possible. (during regular business hours)

Please note: Your insurance's co-payment fees will be determined before your first appointment.
Blue Cross Blue Shield Network E and Network L insurance plans are not accepted.
