  2. After your payment is completed, you'll be redirected to submit your intake documents.

We will review your submission and contact you quickly to schedule your first appointment. (during regular business hours)

Please note: Your insurance copay, coinsurance, and any non-covered fees will be provided to you prior to scheduling, so you'll have a clear picture of your treatment costs. Blue Cross Blue Shield Network E and Network L insurance plans are not accepted at this time. We accept all Medicaid plans but do not accept Medicare plans at this time.
