That’s why we offer a comprehensive income-based sliding scale program for Suboxone treatment, designed for individuals without behavioral health coverage or who cannot afford care, with or without health insurance.

What Is the Sliding Scale Program?

Our sliding scale program adjusts the cost of care based on federal poverty guidelines (FPG) and household income. This ensures that low-income individuals across Tennessee can access affordable, high-quality telemedicine Suboxone treatment without the financial burden.

We proudly serve uninsured, underinsured, and low-income patients, especially those in rural Tennessee who may lack local access to opioid addiction treatment.

Who Can Apply?

  • You do not have behavioral health coverage
  • You cannot afford your current treatment costs (with or without insurance)
  • You are seeking a TeleMAT (Telemedicine Suboxone Program) program and are experiencing financial hardship
  • You are currently enrolled in MAT and experiencing a sudden financial hardship
  • You live in Tennessee and meet income and residency requirements

Required Documents

To apply, please provide:

  • One Proof of Income (pay stubs, bank statement, tax return, benefits letter)
  • One Proof of Tennessee Residency (valid ID, utility bill, voter card, lease, etc.)
  • One Proof of No Behavioral Health Coverage (TennCare denial, letter from insurer)
  • A fully completed application with household size and income information

Special Circumstances Clause

If you’re already receiving treatment and experience a sudden financial emergency, such as job loss or a family crisis, you may apply under our Special Circumstances clause for temporary assistance.

  2. Send a message requesting to apply for the Sliding Scale Program
  3. Our staff will walk you through the short application
  4. If approved, your tier will be assigned and treatment can begin immediately
